Errata to the first edition

• p. 141, Eq. (5.6): remove second "=" sign
• p. 240, Table 8.2: quasi-uniform model: replace r^2 by r^4
• p. 251, Eq. (8.52): r^2 is missing from the argument of the exponent.
• p. 253, Eq. (8.57): z^(2/beta) should appear in the numerator
• p. 256, Eq (8.66): lower bound of the integral is zero.
• p. 324, Eq. (D.5) : The term 2^(n-1)(N-1)!/C^(2n) should appear on the left hand hand side of the equation.
